Applied Care and Development (ACAD) offer an Outreach Support Initiative which is a flexible, open ended programme of care and support to children and young people aged 5 and upwards.
The range of programmes of care and support are needs led and have been effective in areas of crisis management and prevention and keeping families together. The Outreach service can be useful following a 6 week programme of residential care to follow on work and ensure continuity of care. The Outreach service can also provide discreet pieces of work in the family home - for example, strengthening family relationships and preventing the requirement for residential care in the longer term.
The Outreach Support Initiative is adaptable and flexible in its approach, is family inclusive and is available out of hours, evenings and weekends.
ACAD Education offer an outreach service to support pupils with additional support for learning needs. We can support young people who are not in care to help them maintain their mainstream schooling.
We work with other agencies and schools through a multidisciplinary approach to ensure the coordination of individualised programmes of study and support to achieve National Qualifications and develop skills in literacy and numeracy and personal development.
